Ratify new Membership Application form

Greetings board members,

The MWG have created a new membership application form for your consideration.

The key information on the form remains the same apart from

• State has altered to State/Province is no longer mandatory

• Postcode is no longer mandatory

• Duplicated wording has been tidied on the last two questions.

The form now has a number of sections. The key information is up front and then the optional demographics later on. The form should not take more than 5-10 minutes to complete.

• 2. Core information (existing)

• 3. Demographics (new and optional)

• 4. Experience (new and optional)

• 5. Socials (new and optional)

The form has been designed to allow us to understand how our membership has altered over the years, allows us to sift for appropriate members for special events quickly. It is anticipated that we would then send a similar form to our current members to update their details.

The MWG would like to request feedback on the form and any suggestions please.
